What Makes a Changemaker? The Politics of Policy Change in the Obesity Crisis

Obesity rates have nearly tripled worldwide since 1975, placing nearly two billion people at risk for further health conditions ranging from heart disease to severe COVID-19. Though a range of policy tools exist for addressing this health challenge, progress by most governments has been slow, and the issue receives relatively little public attention. My book emphasizes the importance of politics for understanding these dynamics. It does so by studying the obesity problem in Germany and the United States – two known policy laggards – to answer three questions. First, the book asks what explains different institutional arrangements for dealing with this public health challenge, showing how World War II served as a critical juncture that fragmented public health institutions in Germany while strengthening them in the United States. Second, it explores public opinion toward obesity policies, demonstrating how partisan politics and political ideology structure public opinion toward obesity despite the relatively low salience of the issue in the politics of both countries. Finally, it considers why obesity-related policy change occurs when it does, developing a novel typology of policy entrepreneurs to argue that those with more altruistic motives have been central to pushing policy change on this issue. The book uses a multimethod approach involving analysis of original survey data and experiments, internet search data, archival documents, and over 120 elite interviews. It improves understanding of the mechanisms that shape policy change for low salience issues, while also contributing to knowledge about the influence of politics on public health policy.
